{"id":6640,"date":"2021-09-13T21:04:46","date_gmt":"2021-09-13T19:04:46","guid":{"rendered":"https:\/\/meddists.com\/learn\/pre-clinical\/pathology\/respiratory-system\/ards-2\/"},"modified":"2021-09-13T21:05:17","modified_gmt":"2021-09-13T19:05:17","slug":"ards-2","status":"publish","type":"page","link":"https:\/\/meddists.com\/learn\/pre-clinical\/pathology\/respiratory-system\/ards-2\/","title":{"rendered":"ARDS"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">Also known as <strong>adult <\/strong>respiratory distress syndrome.<\/p>\n\n\n<span class=\"block-heading\" id=\"header_1\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\"><sup>1<\/sup>Definition<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_1\">\n\n\n<p class=\"wp-block-paragraph\">An acute respiratory failure is associated with inflammation.<\/p>\n\n\n<\/span><span class=\"block-heading\" id=\"header_2\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Clinical definition<sup>1<\/sup><\/h4>\n<\/span><span class=\"block-content\" id=\"contents_2\">\n\n\n<p class=\"wp-block-paragraph\">The Berlin definition requires all four criteria to be present for the diagnosis of ARDS:<\/p>\n\n\n\n<ol class=\"wp-block-list\"><li>Time &#8212; Within a week of a known clinical insult<\/li><li>Chest imaging &#8212; Bilateral opacities consistent with pulmonary edema<\/li><li>Origin of edema &#8212; Not fully explained by cardiac failure or fluid overload<\/li><li>Oxygenation &#8212; Impaired oxygenation<\/li><\/ol>\n\n\n<\/span><span class=\"block-heading\" id=\"header_3\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">General mechanism<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_3\">\n\n\n<p class=\"wp-block-paragraph\">An acute insult to the airways leads to an inflammatory reaction, resulting in the destruction of alveoli.<\/p>\n\n\n<\/span><span class=\"block-heading\" id=\"header_4\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Cellular mechanism<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_4\">\n\n\n<p class=\"wp-block-paragraph\">The mechanism is based on the cellular reaction during acute inflammation:<\/p>\n\n\n\n<ol class=\"wp-block-list\"><li>An acute insult causes the release of IL-8 and other cytokines (TNF, IL-1) by alveolar macrophages<\/li><li>Endothelial activation and recruitment of neutrophils by chemotaxis<\/li><li>The neutrophils activate and release their products, causing alveolar damage:<ol><li>Epithelial and endothelial damage causing alveolar edema<\/li><li>Loss of surfactant<\/li><\/ol><\/li><li>The damage results in necrosis and formation of&nbsp;<strong>hyaline membranes&nbsp;<\/strong>composed of the necrotized cells, edema fluid, and CT fibers<\/li><li>At the end of the repair (organizing phase), type II pneumocytes proliferate rapidly in order to regenerate the alveolar lining<ol><li>Recall that type I pneumocytes are the squamous cells that line the alveoli, and they are unable to proliferate<\/li><li>Usually, the repair is unsuccessful, due to CT deposition, and the end result is intra-alveolar fibrosis<\/li><\/ol><\/li><\/ol>\n\n\n<\/span><span class=\"block-heading\" id=\"header_5\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Characteristics<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_5\">\n\n\n<ul class=\"wp-block-list\"><li>All ARDS cases lead to bilateral alveolar damage<\/li><li>Severe cases may lead to a life-threatening respiratory insufficiency<\/li><\/ul>\n\n\n<\/span><span class=\"block-heading\" id=\"header_6\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Etiology<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_6\">\n\n\n<p class=\"wp-block-paragraph\">The acute insult is most commonly the result of:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Pneumonia<\/li><li>Sepsis<\/li><li>Shock<\/li><\/ul>\n\n\n<\/span><span class=\"block-heading\" id=\"header_7\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Microscopic morphology<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_7\">\n\n\n<ul class=\"wp-block-list\"><li>Endothelial and epithelial injury<\/li><li>Presence of hyaline membranes<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">1.ARDS D, Ranieri V, Rubenfeld G, et al.
