Definition<\/h4>\n<\/span>\n\n\n

The term is used for the description of abnormal contents within the pleura of the lung, usually accompanied by tissue damage.<\/p>\n\n\n<\/span>\n

General mechanism<\/h4>\n<\/span>\n\n\n

Recall that apart from a few mL of pleural fluid, the pleural space is supposed to be empty.<\/p>\n\n\n<\/span>\n

Pneumothorax<\/h3>\n<\/span>\n\n\n
  • Gas within the pleural cavity<\/li>
  • Usually due to an injury<\/li><\/ul>\n\n\n<\/span>\n

    Pleural effusion<\/h3>\n<\/span>\n\n\n

    Fluids in the pleural space.<\/p>\n\n\n\n

    • Exudate<\/strong> (high protein content)<\/li>
    • Transudate<\/strong> (low protein content)<\/li>
    • Fibrinous <\/strong>(fibrin-rich)<\/li>
    • Hemothorax <\/strong>(bloody)<\/li>
    • Chylothorax <\/strong>(concentrated, dehydrated lymphatic fluid)<\/li><\/ul>\n\n\n<\/span>\n

      Exudate<\/h4>\n<\/span>\n\n\n

      Pleural exudate can be due to:<\/p>\n\n\n\n

      • Microbial infection<\/strong> (suppurative pleuritis)<\/li>
      • Viral infection<\/strong><\/li>
      • Cancer<\/strong><\/li>
      • Pulmonary infarction<\/strong><\/li><\/ul>\n\n\n<\/span>\n

        Transudate<\/h4>\n<\/span>\n\n\n
        • Usually referred to as hydrothorax<\/li>
        • Most often will be absorbed without any side-effect<\/li><\/ul>\n\n\n<\/span>\n

          Primary Lesions<\/h3>\n<\/span>\n\n\n
          • Bacterial infections<\/li>
          • Neoplasms<\/li><\/ul>\n\n\n<\/span>\n

            Neoplasms<\/h4>\n<\/span>\n\n\n

            Primary pleural neoplasms are relatively rare.<\/p>\n\n\n\n

            • Malignant mesothelioma<\/strong>
              • Tumor of mesothelial cells taking one of the following forms:
                • Epithelial<\/strong><\/li>
                • Sarcomatoid<\/strong><\/li>
                • Biphasic<\/strong><\/li><\/ul><\/li>
                • Closely correlated to asbestos exposure
                  • Usually takes many years to develop<\/li><\/ul><\/li>
                  • Pleural fibrosis and plaque formation<\/li><\/ul><\/li>
                  • Solitary fibrous tumor<\/strong>
                    • Mostly benign but have a potential for malignancy<\/li>
                    • Well-circumscribed, and often large<\/li>
                    • Grows slowly<\/li>
                    • Fibrous stroma with a low number of mitotic figures<\/li>
                    • Cells are CD34 and BCL-2 positive<\/li><\/ul><\/li><\/ul>\n<\/span>
