{"id":5353,"date":"2021-05-15T23:53:42","date_gmt":"2021-05-15T21:53:42","guid":{"rendered":"https:\/\/meddists.com\/learn\/clinical\/hematology\/hematological-oncology\/indolent-lymphomas-follicular-malt-hairy-cell\/"},"modified":"2021-05-27T15:30:18","modified_gmt":"2021-05-27T13:30:18","slug":"indolent-lymphomas-follicular-malt-hairy-cell","status":"publish","type":"page","link":"https:\/\/meddists.com\/learn\/clinical\/internal-medicine\/hematology\/oncological-disorders\/lymphomas\/indolent-lymphomas-follicular-malt-hairy-cell\/","title":{"rendered":"Indolent lymphomas (follicular, MALT, hairy-cell)"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\"><em>Make sure you cover the <a href=\"https:\/\/meddists.com\/learn\/clinical\/hematology\/oncological-disorders\/lymphomas\/\">lymphomas<\/a> article first.<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><div class=\"intro\">Indolent lymphomas are those lymphomas that are slow-growing and progressing, but they are generally considered incurable.<\/div><\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>They include:<ul><li>CLL\/SLL<\/li><li>Follicular lymphoma<\/li><\/ul><ul><li>Marginal zone lymphoma<\/li><\/ul><\/li><li>Generally, treatments include:<ul><li>CHOP +- R<\/li><li>CVP +- R<\/li><\/ul><\/li><\/ul>\n\n\n<span class=\"block-heading\" id=\"header_1\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Follicular lymphoma<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_1\">\n\n\n<p class=\"wp-block-paragraph\">Follicular lymphoma is B-cell lymphoma found in the germinal center.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>2nd-most common NHL, appearing in older adult smokers.<\/li><li>Histologically, the entire lymph node is packed full of lymphoid follicles (occupying the medulla as well); graded I-III based on the number of <strong>centrocytes<\/strong> and <strong>centroblasts<\/strong>.<\/li><li>Associated with t(14;18) with BCL2 overexpression.<\/li><li>Presents with painless lymph node enlargement.<\/li><li>Can progress to DLBCL.<\/li><li>Treatment:<ul><li>First line: R-CVP, R-CHOP, R-benda, R, G-benda<\/li><li>Maintenance: R alone<\/li><\/ul><\/li><\/ul>\n\n\n<\/span><span class=\"block-heading\" id=\"header_2\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">MALT lymphoma<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_2\">\n\n\n<p class=\"wp-block-paragraph\">MALT lymphoma is a type of <strong>extranodal marginal zone lymphoma<\/strong> arising in the mucosal tissues. <\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>It resembles the mucosa-associated lymphatic tissue (hence the name).<\/li><li>It is thought to arise as lymphocyte hyperplasia due to chronic inflammation, undergoing oncological genetic changes (most often of the stomach caused by H. pylori).<\/li><li>In the stomach, it appears similar to a gastric ulcer and behaves in a similar way (bleeding).<\/li><li>Treatment includes eradication of the cause of inflammation and <strong>R-CHOP<\/strong> to treat the lymphoma.<ul><li>Treatment for H. pylori: amoxicillin, clarithromycin, metronidazole, and omeprazole.<\/li><\/ul><\/li><\/ul>\n\n\n<\/span><span class=\"block-heading\" id=\"header_3\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Hairy-cell leukemia<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_3\">\n\n\n<p class=\"wp-block-paragraph\">Hairy-cell leukemia is a rare form of an indolent B-cell lymphoproliferative disorder that is typically classified under CLL. <\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Thought to arise from memory B cells of the marginal zone; has similar characteristics to MZL.<\/li><li>The cells have hairlike (&#8220;hairy&#8221;) cytoplasmic projections; relatively exclusive surface markers include CD11c and CD103.<\/li><li>Presents with hepatosplenomegaly, bone marrow infiltration, and monocytopenia, but typically without lymph node enlargement.<\/li><li>Presents with BRAF mutation.<\/li><li>Extremely sensitive to chemotherapy; excellent prognosis.<\/li><li>Treatment includes rituximab + cladribine or pentostatin.<\/li><\/ul>\n<\/span><div id=\"the_titles\" style=\"display:none;\"><h4 class=\"wp-block-heading\" class=\"wp-block-heading\">Follicular lymphoma<\/h4><h4 class=\"wp-block-heading\" class=\"wp-block-heading\">MALT lymphoma<\/h4><h4 class=\"wp-block-heading\" class=\"wp-block-heading\">Hairy-cell leukemia<\/h4><\/div>","protected":false},"excerpt":{"rendered":"<p>Make sure you cover the lymphomas article first.
