{"id":5266,"date":"2021-05-13T19:33:47","date_gmt":"2021-05-13T17:33:47","guid":{"rendered":"https:\/\/meddists.com\/learn\/clinical\/hematology\/anemias\/hemolytic-anemias\/g6pd-deficiency-anemia\/"},"modified":"2021-05-22T18:54:09","modified_gmt":"2021-05-22T16:54:09","slug":"g6pd-deficiency-anemia","status":"publish","type":"page","link":"https:\/\/meddists.com\/learn\/clinical\/internal-medicine\/hematology\/anemias\/hemolytic-anemias\/g6pd-deficiency-anemia\/","title":{"rendered":"G6PD deficiency anemia"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\"><div class=\"intro\">G6PD deficiency is an X-linked recessive condition with acute attacks of hemolytic anemia due to hemolysis triggered by oxidative stress.<\/div><\/p>\n\n\n<span class=\"block-heading\" id=\"header_1\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Characteristics<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_1\">\n\n\n<ul class=\"wp-block-list\"><li>Normal blood count between the acute attacks.<\/li><li>The oxidative stress is typically caused by an infection, many groups of drugs (including certain antibiotics and antimalarials), and fava beans.<\/li><li>The acute attack occurs within several hours up to 2 days after the exposure.<\/li><\/ul>\n\n\n<\/span><span class=\"block-heading\" id=\"header_2\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Pathophysiology<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_2\">\n\n\n<p class=\"wp-block-paragraph\">The glucose-6-phosphate-dehydrogenase enzyme is responsible for the regeneration of glutathione, an important antioxidant that helps RBCs handle ROS. As RBCs cannot repair themselves, they rely on glutathione to prevent oxidative damage.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In G6PD, RBCs cannot regenerate reduced glutathione, leading to increased vulnerability of the cells to oxidative damage.<\/p>\n\n\n<\/span><span class=\"block-heading\" id=\"header_3\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Clinical presentation<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_3\">\n\n\n<p class=\"wp-block-paragraph\">During an acute attack:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Jaundice<\/li><li>Abdominal pain, diarrhea, and vomiting<\/li><li>Hemoglobinuria (coca-cola urine)<\/li><\/ul>\n\n\n<\/span><span class=\"block-heading\" id=\"header_4\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Diagnosis<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_4\">\n\n\n<ul class=\"wp-block-list\"><li>Reticulocyte count (increased)<\/li><li>Haptoglobin (absent)<\/li><li>Blood smear (during the acute attack)<ul><li>Heinz bodies (intracellular RBC inclusions composed of denatured hemoglobin) visualized as blue aggregations using crystal violet or new methylene blue stain, especially in asplenic patients.<\/li><li>&#8220;Bite&#8221; and &#8220;blister&#8221; cells resulting from splenic macrophage removal of the Heinz bodies from RBCs.<\/li><\/ul><\/li><li>G6PD level (may give a false negative during an acute attack, but low levels after it)<\/li><\/ul>\n\n\n<\/span><span class=\"block-heading\" id=\"header_5\">\n<h4 class=\"wp-block-heading\" class=\"wp-block-heading\" class=\"title_collection title1\">Treatment<\/h4>\n<\/span><span class=\"block-content\" id=\"contents_5\">\n\n\n<ul class=\"wp-block-list\"><li>Immediate removal of the causative agent <\/li><li>Adequate hydration and osmotic diuretics<\/li><li>Blood transfusion in severe anemia<\/li><\/ul>\n<\/span><div id=\"the_titles\" style=\"display:none;\"><h4 class=\"wp-block-heading\" class=\"wp-block-heading\">Characteristics<\/h4><h4 class=\"wp-block-heading\" class=\"wp-block-heading\">Pathophysiology<\/h4><h4 class=\"wp-block-heading\" class=\"wp-block-heading\">Clinical presentation<\/h4><h4 class=\"wp-block-heading\" class=\"wp-block-heading\">Diagnosis<\/h4><h4 class=\"wp-block-heading\" class=\"wp-block-heading\">Treatment<\/h4><\/div>","protected":false},"excerpt":{"rendered":"<p>Characteristics Normal blood count between the acute attacks.
